Occupational safety and health (OSH) is an important aspect of machining activities because the use of machinery involves various hazards that may cause occupational accidents. This study aimed to identify hazards, assess risk levels, analyze the causes of hazards, determine risk controls, and develop a web-based OSH risk management information system in the Production Process Laboratory of the Industrial Engineering Study Program. The methods included hazard identification on milling machines, lathes, and 3D printing machines, risk assessment using the AS/NZS 4360:2004 method, root cause analysis using the 5 Whys method, and information system development using the Laravel framework and MySQL database. The results showed that 166 potential hazards were identified throughout the machining processes. Risk assessment identified four risk categories: acceptable, priority 3, substantial, and priority 1, with acceptable and priority 3 being the most frequently identified categories. The 5 Whys analysis indicated that the main causes included the absence of machine-specific SOPs for personal protective equipment (PPE), the lack of written work instructions, and limited availability of work aids. Recommended risk controls included developing SOPs and work instructions, regulating work methods, and providing PPE according to the hazards associated with each work process. The developed system also fulfilled the main functional requirements based on Black Box Testing.
