PREDIKTOR KLINIS KETERLAMBATAN DIAGNOSIS HIV PADA PRESENTASI NEUROPSIKIATRI: SUATU TINJAUAN SISTEMATIS
Abstract
Neuropsychiatric symptoms are repeatedly documented as presentations that precede diagnosis of HIV infection, yet the clinical factors predicting diagnostic delay in this subgroup are not well characterised. This systematic review aimed to identify and synthesise clinical factors associated with delayed HIV diagnosis, and to assess how neuropsychiatric symptoms have been incorporated into this evidence base. Methods: A structured literature search of PubMed/MEDLINE was conducted for publications from 2021-2026, following the PRISMA 2020 framework; odds ratios and 95% confidence intervals were extracted verbatim. Results: Older age, male sex, heterosexual transmission, provider-initiated testing, and diagnosis in a medical institution rather than through voluntary counselling were consistently associated with delayed HIV diagnosis. Neuropsychiatric presentations were repeatedly informative in case-based literature but were not identified as a coded predictor in any quantitative model reviewed. Conclusion: Established clinical predictors of delayed HIV diagnosis are quantitatively well documented, but their interaction with neuropsychiatric presentation remains an evidence gap that future studies, including in Pegunungan Bintang Regency, Papua, Indonesia, should address directly. Keywords
