WANITA 28 TAHUN P1A2 PASCA LAPARATOMI, SALPHYNGEKTOMI SINISTRA ATAS INDIKASI RUPTURE TUBA PARS AMPULLARIS SINISTRA (KEHAMILAN EKTOPIK TERGANGGU)
Abstract
Background: Ruptured Ectopic Pregnancy is a life-threatening obstetric emergency where a fertilized ovum implants and develops outside the uterine cavity, most commonly in the ampullary region of the fallopian tube, and subsequently ruptures. This condition leads to massive intra-abdominal hemorrhage. Early diagnosis and immediate surgical intervention are critical to prevent maternal morbidity and mortality. Case A 28-year-old female patient (P1A2) presented to the Emergency Department with a chief complaint of severe lower abdominal pain (Wong-Baker scale 8/10), cold sweats, and profound weakness. The patient had a positive home pregnancy test with an estimated gestational age of 8 weeks. Physical examination revealed an alert general condition but with severe pain distress, blood pressure of 104/64 mmHg, tachycardic heart rate of 112 beats/minute, and a positive cervical motion tenderness on gynecological examination. The patient was promptly diagnosed with a suspected ruptured ectopic pregnancy and underwent an emergency laparotomy involving a left salpingectomy indicated by a ruptured left ampullary fallopian tube. Conclusion: Ruptured ectopic pregnancy due to an ampullary tube rupture requires rapid diagnosis through history taking, physical examination, and investigations. Emergency laparotomy and salpingectomy serve as the definitive life-saving management in cases of active rupture and hemodynamic threat. Strict postoperative monitoring ensures successful patient recovery. Keywords
